If you have received a referral from your GP via ZorgDomein, you will automatically be placed on the waiting list. If you have received the referral on paper, you must still fill in the registration form.
REGISTER YOURSELF:
You can fill out the registration form. When I receive your registration, you will be put on the waiting list.
A referral letter is required for reimbursement of costs through health insurance.
